Liability Waiver
Brazilian Jiu Jitsu, wrestling, and martial arts are potentially dangerous activities. Bumps, bruises, scrapes, scratches, and soreness are commonplace, and most students will encounter this sort of injury from time to time in their training. More serious injuries are possible, including sprains, strains, twists, cramps, and injuries of similar magnitude, and students can expect to encounter these injuries infrequently. Although rare, the possibility of more serious injury exists.
I understand the above statement of risk, and I understand the rights and responsibilities of students. I assume responsibility for my own safety (or the safety of my child), understanding and accepting the risks involved with martial arts training. By assuming this risk, I completely absolve all instructors, staff, students, guests, visitors, landlords, managements, and any and all other parties of liability for my harm, unless intentionally caused in criminal conduct.I have read the document in its entirety, and I understand the complete content of it. I agree to abide by the terms of it.
Media Release:
I hereby authorize Magalit Combat Sports LLC, its employees, and assigns ("Magalit Brazilian Jiu Jitsu") to photograph, film, videotape or record me or my minor child and use my or my minor child's name, voice and/or likeness in such photograph, film, video, or other recording taken or made by Magalit Combat Sports LLC or provided by me.
I understand that any photograph, sound recording, motion picture, video or other recording taken of or provided by me under this assignment (“Material”) can be used for any purpose including marketing and social media posts.
I hereby assign to Magalit Combat Sports LLC all rights, title, and interest, including copyright, in and to any and all such Material and understand and agree that I will not receive monetary payment for any use of the Material by Magalit Combat Sports LLC. I waive any right to inspect or to approve any use by Magalit Combat Sports of any such Material.
By submitting this form, I acknowledge that I have freely and voluntarily entered into this agreement on behalf of myself or my minor child.
